All documents must be completed, signed and dated. Referral Form required prior to treatment for motor vehicle accident claims. Please refer here for more information.
Minor Consent Form
For potential clients under 18 please have a parent or guardian review and fill out this form. First time clients 16 and under must have a parent or guardian on the premises. Pediatric clients must have parent or guardian in the treatment room for the duration of treatment.
